I need an MOU or other legal agreement that will allow Chronically Informed to become a project under the Chronic Care Collaborative, a organization that has 501(c)3 status. I want to be sure that I maintain all creative and fiscal control of the project, that I retain legal rights to all things related the project, including intellectual property, that they don't need to give me money and vice versa, and that Chronically Informed can terminate work with the Chronic Care Collaborative at any time. Chronically Informed can be found at chronicallyinformed.org.

Chronically Informed showcases videos that share stories of success and practical techniques for navigating the medical system. These videos create hope for patients and advance health and wellbeing by helping them understand that others have found ways to navigate the medical system and inspire patients to use those pathways for themselves. The mission of Chronically Informed is to create hope for people with chronic illness, help patients partner with their providers, and improve health equity for patients. Chronically Informed does this by empowering patients with chronic health conditions and their caregivers to navigate the medical system more effectively through a platform that is easily accessible to everyone. We are fiscally sponsored by The Archer Group dba The Fund for a Healthier Colorado.
