Thank you for considering helping us! We currently have a fiscal sponsor agreement with one nonprofit and an MOU with a different organization. We are trying to figure out if we still need the MOU and with whom we should have MOUs in the future. While this will likely be a relatively easy project for you, it will help us immensely! Chronically Informed showcases videos that share stories of success and practical techniques for navigating the medical system. These videos create hope for patients and advance health and wellbeing by helping them understand that others have found ways to navigate the medical system and inspire patients to use those pathways for themselves. The mission of Chronically Informed is to create hope for people with chronic illness, help patients partner with their providers, and improve health equity for patients. Chronically Informed does this by empowering patients with chronic health conditions and their caregivers to navigate the medical system more effectively through a platform that is easily accessible to everyone. We are fiscally sponsored by The Archer Group dba The Fund for a Healthier Colorado.
